Used to happen with me too. There was a way to fix this but I don't remember it now. However, you can disable the visibility and turn it back on(Toggle attachment visibility), it will be fine.

You can try this tho,
Save attachment
Automatic attach on spawn: Enable
Attachment visibility: Disable
/kill
Attachment visibility: Enable

I'm not sure, but this might work out.
