I don't know for sure because I chose to leave the game for a years(+) after that incident. I think this happened because previously I played on a mobile device. However, I also investigated it a little and asked several players at that time. They said relog could solve it, but basicly if it continues to happen, it seems like it will trigger players concerns. Now I'm back, but i've never encountered a bug like before... Maybe there was a trigger that caused this to happen. Next time I'll delve deeper into it and explain it again here if necessary, thanks.

Do the icons just randomly appear while playing? Or does it instantly appear when joining the server?

Appears when you are AFK and when you return.
