This happens, when no icons where found or loaded.
1. Please make sure, that one of the icon packs is selected under Settings - Options -> Design.
These zip file must be there to get the icons.
default , default_mono_2014, default_colored_2014

2. Check that the files arn't missing on your hard drive.

C:\Program Files\TeamSpeak 3 Client\gfx\default.zip
C:\Program Files\TeamSpeak 3 Client\gfx\default_mono_2014.zip
C:\Program Files\TeamSpeak 3 Client\gfx\default_colored_2014.zip

3. If all does not resolve your issue, then download and install the lcinet form the official download page
