Previous Group: The previous official group in which you were.
Activity: The Total number of hours you have played in this server.
Joined: When did you join the server (How many days ago?: 100, 600, etc)
Proof of Status: Link of your in-game /stats picture
Time Zone & Country: Mention your GMT and Country
Reason: I want to join terrorists because its awesome..

That image of yours look like a signature( its cool tho).
The picture in the template is professional stamp-type picture and is better imo.
