Custom application voting templates

Greetings,

there has been a suggestion about an official voting template, and since many of you are designing your own voting templates, why not have a topic in our pub about it. Voting using templates is optional, that's why you may design your own and post it here for others to use, if you wish.

You are allowed to post anything in any format (comment on others etc.), but when posting a template I'd suggest something like this, to keep the topic clean:
