Everything matches perfectly. (By everything I mean some stuff which are classified and cannot be mentioned here).
You are accountable for anything that is required to log into a samp server (Such as username, Own PC, etc..). If you are in a cyber cafe , Sorry I can't help you with that.
Your ban is permanent and will not be lifted anytime in future.
Edit (Updated): User claims that he were accessing the server from a PC in his home.
So, In that case the guy that he said copied and impersonated his name (Moz3eg), must have accessed the server from his PC.
